INSEMINATION: A GENTLE PATH TO PARENTHOOD

Insemination is a well-established and minimally invasive fertility treatment. It offers a gentle and effective option for both heterosexual and same-sex couples hoping to start a family.

During insemination, sperm cells are placed directly into the uterus at the most fertile point in the woman’s cycle. This increases the likelihood of fertilisation by helping more sperm reach the egg than would naturally occur.

Insemination: gentle solution for all couples with a desire to have children

Semen transmissions are just as possible for heterosexual couples as they are for lesbians. The treatment is a gentle way of fulfilling your desire for children.

We differntiate in:

  • Homologous Insemination: The sperm used is from the woman’s partner.
  • Heterologous Insemination: Donor sperm is used. This option is suitable not only for lesbian couples but also for women whose male partners are infertile. A notarised consent form is required, confirming agreement to the use of donor sperm. After birth, both partners are legally recognised as parents, regardless of their gender or marital status.

In the Wunschkind Klinik we carry out the so-called Intrauterine Insemination (IUI): We transfer the sperm directly into the uterus of the woman. The success of an insemination depends, in large part, on the sperm being transmitted at the optimal time in the woman's cycle. This is the case when the follicle is between 17 and 22 millimeters in size.

Depending on the procedure we distinguish in:

  • Natural cycle IUI: We monitor your cycle using ultrasound to track follicle development and predict ovulation.
  • Stimulated cycle IUI: In some cases, we use hormone therapy to increase the number of mature follicles. Once the follicles are ready, ovulation is induced and insemination is timed precisely.

The right time for insemination is crucial for the chances of success. Therefore, couples who want to undergo insemination should be flexible in their time. An essential prerequisite for any insemination is that the Fallopian tubes are open. We clarify this through our diagnosis.

We use either fresh or cryopreserved sperm, from your partner or a donor. Fresh samples are collected at our clinic in a private, comfortable room. In certain cases, sperm must be surgically retrieved directly from the testicles — for this, we collaborate with experienced specialists.

The sample is then prepared in our laboratory, where we select the most motile sperm for insemination. The sperm is introduced into the uterus via catheter, and from there, nature takes over. Three weeks after the procedure, we perform a pregnancy test. If positive, the baby’s heartbeat can usually be seen on ultrasound three weeks later.

The success of insemination depends on several factors:

  • Age of the woman
  • Her reproductive health
  • Quantity and quality of the sperm

Often, multiple attempts are required. The average pregnancy rate per cycle is between 12% and 20%, depending on age. After one year, 80% of couples will have conceived. If pregnancy hasn’t occurred after three or four cycles, we typically recommend moving to IVF.

Insemination is generally low risk, but there are a few possible challenges:

  • Physical strain from hormone therapy
  • Mild risk of infection
  • Emotional stress, particularly if treatment is prolonged

At the most fertile time in your cycle, sperm is placed directly into the uterus to increase the chance of fertilisation.

In these situations, we advise you to semen transfer:

  • Low sperm count or poor motility
  • Erectile or ejaculation problems
  • Issues with sperm reaching the uterus
  • Cervical mucus blocking sperm
  • Physical conditions preventing intercourse

Yes, though minimal. Risks may include:

  • Hormonal side effects (e.g. mood changes)

Pregnancy rates are between 12–15% per cycle. Lesbian couples often see slightly higher success rates due to fewer underlying reproductive issues like PCOS or blocked fallopian tubes.

It varies. Most couples require several attempts.

Naturally – after insemination, the sperm travels to the fallopian tubes, where it may meet and fertilise an egg.

We wait. A pregnancy test is done about three weeks later. If successful, a heartbeat can usually be seen three weeks after that.

Flexibility is important. Insemination is timed precisely to the woman’s cycle. Ideally, fresh sperm is used and collected at our clinic.
